A certain physician of Ormond Beach, Florida (Volusia County) used one of his patient's information to fraudulently obtain controlled substances. The patient found out when her insurance company raised her premium.  The physician received his new silver bangles (handcuffs) earlier today, July 1, 2011.

Insurance companies send out EOBs or Explanation of Benefit documents, which show the dates that health care services were rendered, codes or summary of those health care services, as well as the name of the doctor or health care provider. Just because it reads, "THIS IS NOT A BILL," does not mean it is unimportant. When we don't report fraud, we pay in the end.
